CS Form 100 (Rev 2023)	Print back-to-back on legal-size (8.5 × 13 in).
Four passport-size photos	White background; taken ≤3 months ago; hand-written name tag & signature 1 inch below chin.
Valid I.D. (orig + photocopy)	Any on CSC list—Driver’s License, PhilSys, Passport, PRC, UMID, Voter’s ID, etc.
₱500 exam fee	Cash only at CSC cashier.
Optional docs	PSA Birth Cert (if DOB not on I.D.); Medical Cert (for PWD).

Book the earliest online appointment—the CSC portal throttles after 10 AM.
Pick your clearest I.D.; blurry company cards cause the most rejections.
Paperclip (don’t staple) your photos—evaluators remove staples and it slows the line.
Fill CS Form 100 tonight—on-site scribbling adds ~20 minutes.
Monitor regional CSC Facebook pages for “Slot Full” notices; have a Plan B field office.
Screenshot the payment stub immediately; some receipts fade in weeks.
